Use this form to report the loss of your Blue Badge

Before you complete this form, you must first report your Blue Badge as lost via Report my loss. The Police no longer take reports or provide loss property reference numbers for items lost.

You will be charged a fee to register your Blue Badge as lost with this service. Once you have completed the online loss report, you will need to provide a copy of it with this form.
