Apply for the scheme
The scheme is free and you can apply for it if you are a:
- premises licence holder
- designated premises supervisor
- premises owner
- premises manager.
You'll need to send a completed self-assessment application form to us for consideration (see 'Useful documents' below). Complete section one of the form and either section two or three depending on the type of business you run.
Please carefully read the criteria set out in the form and use the 'Evidence' section to describe how you're able to meet the standards required for membership of the scheme.
